The QwikForms CD-ROM is being sold without a sleeve. While writing a review
for it and exploring the disc I made a sleeve. This postscript version
includes the fonts so the CD is not needed to print the cover (but what's
the use then?). It seems like some postscript printers (i.e. HP laserjet
4M) do not accept bitmaps with a transperant background; this will make
this sleeve less attrictive but still very much usable.
